Position Overview:

The Outpatient Assessor conducts assessments primarily via telehealth, face to face if needed, to determine proper level of care for substance abuse and mental health programs. This individual acts both as a liaison between the prospective client and designated OP office to ensure warm hand off between levels of care as well as the clinical first contact for outside referrals. original Outpatient clients

Specific Responsibilities:

  • Support collaboration and communication with RCA inpatient staff and RCA outpatient staff for scheduling stepdown assessments.
  • Work with Mission Center personnel for the Intake, Assessment and scheduling of new Outpatient clients.
  • Schedule clients and ensure financial responsibilities are reviewed.
  • Help clients create individualized treatment plan goals and objectives within designated timeframes.
  • Update assessment trackers daily and communicate any noticeable trends to supervisor.
  • Work collectively with the clinical team to engage, educate, and coordinate client care with the client, their identified supports, and community providers to ensure all services prescribed are implemented.
  • Actively participate in the development and assessment of individual treatment and service plans.
  • Complete intake Biopsychosocial Assessments, including all electronic forms required by RCA and/or third-party payors.
  • Evaluate clients’ mental and physical health, addiction severity, and/or problem behavior and assess their readiness for treatment and articulates clinical rationale for recommended level of care.
  • Review and recommend treatment options with clients, including their families, if willing.
  • Utilize recognized evidence based therapeutic modalities, such as CBT, DBT, 12-Step, psychoanalytic, motivational interviewing, and stages of change.
  • Is familiar with Medication Assisted Treatment (MAT) and can objectively discuss options with clients.
  • Exhibit knowledge of all continuums of care, including admission assessment and continued stay criteria.
  • Attend treatment team and review new client clinicals with treatment team.
  • Act as an advocate for all clients when working with external providers and speaking on their behalf.
  • Performs Utilization Review tasks, completes utilization reviews, provides information at the request of other members, and communicates all utilization issues that may compromise client’s treatment immediately to treatment team.
  • Document all work in the RCA Electronic Health Record, Avatar.

Education and Experience:

  • Master’s degree in Social Work, Education, Psychology, Mental Health Counseling, or another related human services field required.
  • Licensure as LADC-I, LADC-II, LMHC, LCSW, LPC or LMFT required.
  • Prior experience in a behavioral healthcare or substance abuse settings serving culturally diverse populations required.
  • Experience with Electronic Health Record (EHR) preferred.
  • Excellent verbal and written communication skills required.
  • Excellent organizational skills with the ability to prioritize workload and meet deadlines.
  • Must be able to work in a fast-paced environment with demonstrated ability to juggle multiple competing tasks and demands.
  • Exceptional analytical and problem-solving skills.
  • Proven ability to handle multiple projects under pressure.
  • Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint).
  • Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the clinical interventions.
  • Understands need for and always maintains appropriate confidentiality when interacting with clients, residents, families, visitors, referral sources and all other contacts.
  • Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, clients, residents, visitors, families, and referral sources.
  • Knowledge of ASAM criteria, dual diagnosis and experience working with the eating disorder population.
