Position Overview: The Recovery Care Coordinator is responsible for contacting identified patients post discharge and identifying the needs of the patient and family facilitating a smooth and safe transition from their current level of care. Assures quality clinical care by reviewing and coordinating discharge plan by collaborating with clinical team on plan of care. Identifies patient/discharge needs to assure that both the patient and/or family receive support with resources and placement assistance. The Recovery Care Coordinator is actively involved in helping clients and families, as needed, to identify aftercare needs following inpatient treatment for outpatient treatment services through RCA. This person will operate as a member of the multidisciplinary treatment team by working closely with referral sources, inpatient staff, and the outpatient treatment team. This individual will act as an advocate for supporting the client’s post treatment plan to encourage compliance.



Specific Responsibilities:

  • Ensures 100% of identified patient population is contacted within designated time frame from discharge from treatment
  • Reviews discharge plan with patient and/or family based and ensures compliance with the discharge plan
  • Reviews patient’s goals, psychosocial, environmental, and discharge planning needs, and financial status
  • Supports compliance and monitors the discharge plan through an interdisciplinary team process in conjunction with patient and family in both external and internal settings
  • Communicates effectively with patients, families, medical staff, nursing staff, other caregivers, and third-party payors as necessary
  • Liaison between physicians, patients, healthcare providers, caregivers, and payors to orchestrate, coordinate and act as facilitator of the patient’s post-acute care
  • Determines alternatives for post-acute care to care team and patient / family
  • Utilizes recognized evidence based therapeutic modalities, such as motivational interviewing and stages of change as appropriate.
  • If necessary, follows through with finalizing all aspects of the discharge plan including facilitating hand-off communication between sending and receiving nurses, and confirming transportation / transfer arrangements
  • Maintains an ongoing relationship with patients, families, and the health care team through verbal and written communication to facilitate appropriate and timely progression along the continuum for assigned areas or upon consulted patients
  • Ensures patients/families are aware of and in agreement with all post-acute services, including any possible/actual associated out of pocket costs to the patient and document such in the medical record
  • Maintains timely and up to date documentation of activities in EMR/ Avatar
  • Uses appropriate channels of communication when initiating a change, reporting information, or addressing a concern

* Communicates and documents patient’s progress toward their recovery.
* Maintains the patient’s comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning).
* Works collaboratively with the clinical team to engage, educate, communicate, and coordinate care with patient, their family, behavioral and general medical and dental health care providers, community resources and others to ensure that all services prescribed in the Continuing Care Plan are implemented.
* Provides assistance in maintaining, monitoring and modifying covered behavioral health services.
* Seeks out necessary resources other than covered services to meet basic needs.
* Ensures all appropriate referrals for identified services on the service plan are made and coordinated with service providers via contracted network providers or community resources; Ensures all covered services identified on the service plan are evaluated and updated monthly.
* Ensures documentation adheres to all regulatory timeframes and requirements.

Minimum Qualifications:
* Bachelor’s Degree in a social work field or equivalent combination of education, training, and/or experience.
* Minimum of one (1) year experience in Behavioral Health or Psychiatric.
* Knowledge of health care, detoxification process, addiction, co-occurring disorders, DSM Criteria, and terminology.

* Previous discharge planning experience and/or community resource development expertise is preferred.
* Ability to read and interpret written information; write clearly and informatively; edits work for spelling and grammar. Ability to speak clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions; demonstrates group presentation skills; and participates in meetings.
* Working knowledge of Microsoft Word, Excel, and Outlook.

Work Environment: This job operates in a professional office environment or virtual. This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets. The noise level in the work environment is usually moderate.


Physical Demands: While performing the duties of this position, the employee is regularly required to talk or hear. The employee frequently is required to use hands to finger, handle or feel objects, tools or controls. The employee is occasionally required to stand; walk; sit; reach with hands and arms; climb or balance; and stoop, kneel, crouch or crawl. The employee must occasionally lift and/or move objects up to 25 pounds. Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision and the ability to adjust focus.


Travel: Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected.
